TL084H: Input gets shorted to supply

Hi there,
I am using TL084H op-amp for low pass filtering a signal and around 1 in each 20 boards has a problem and the problem is that the positive input of U6AA becomes tied to either positive or negative supply. The input signal is a square wave which with amplitude of voltage rails but never goes beyond the supply voltages to burn the protection diodes on the inputs. Although the input is not in recommended common mode range but has not exceeded the maximum rating too. Can you give me a hint why is this happening? Is it due to input voltage that goes beyond recommended common mode range or some thing else is causing the issue?
